Is this in reference to when someone steps on the back of one's shoe and the heel part of the shoe gets stuck below one's actual heel?
If so, it's just a reference to automobiles in that it slows you down, and you have to stop to adjust the shoe (or "change your tire").

When I was a young lad in the '70's... a "flat tire" was given by stepping on your friends heel as he walked, so as to pull the shoe off the heel.

I guess this practice went away in the late 80's when sneakers started costing more that my first RENT !!
