Be as blatant with her as she has been with you; tell her you have tried to be nice about it as you didn't want to hurt her feelings, but she hasn't taken the hint so you leave her no choice but to spell it out for her: You are not physically attracted to her, you don't want to go out with her, you don't want her to touch you ever again and if she does, it leaves you no choice but to report her for sexual harrassment. If she has your home phone, e-mail, IM put blocks for her number/name/address on all of them.
If you can't bring yourself to do this, then go your boss and tell the boss what is happening and say that you would like it handled informally, meaning not a formal write up for her, just that you want her to stop. Your boss will have to take your complaint seriously and address the issue with her.

I think at that point, if she is touching you, then you should tell your boss that she's making you umcomfortable... you don't have to use the language "sexual harrassment," unless you're comfortable... even if she's just annoying in the work place, that hinders your job preformance and should be taken to your Human Resources dept.
If she then keeps trying to contact you outside of work, get a restraining order... being stalked is scary.

Just go to your boss and say you're being sexually harassed. Seriously, what she's doing is very wrong and you need to report her as soon as possible. Don't even tell her you're going to do it.

Tell her you're not interested and to leave you alone and that if she doesn't leave you alone you are going to report her for sexual harassment. Then follow through with that if she doesn't stop.

OK, this may sound a little weird, but do the opposite of what you have been doing. Tell her you want/need/love her, tell her she is the only one for you, and that your world revolves around her. Give her flowers, and chocolates, and really try to gush her up. I bet she will run away, thinking your a huge wuss.

Ignore her. Even be a bit nasty to her if you want. Just don't give her what she wants.

tell her that you're sorry but she don't have the right "equipment", you can be friends but she has to respect your wishes. Good luck and hope you feel better.
