He did for a very brief time. It started at the 1995 King of the Ring, when Mabel defeated the Undertaker en route to winning King of the Ring. Mabel would further anger 'Taker when he planted a legdrop across the Undertaker's face, forcing "the Phenom" to wear a protective "Phantom of the Opera"-like mask. Video in the source link.

A FEW YEARS AGO, MABEL LANDED ON HIS FACE AND YOU COULD HARDLY SEE THE UNDERTAKER UNDER MABEL'S A*S! IT WAS HORRIBLE, IF YOU LISTENED CAREFULLY IN THE PLAYBACK YOU CAN HEAR MABEL WHISPER TO "TAKER" NOT TO DO IT,HE WAS'NT POSITIONED RIGHT OR SOMETHING,ANYWAY WHEN THE UNDERTAKER REACHED UNDER MABEL TO PICK HIM UP(i don't know what he was thinking...) HE DID'NT HAVE HIM AND MABEL WAS GRASPING AT THE ROPE TO TRY NOT TO FALL BUT IT WAS TOO LATE! BONES IN MARK'S FACE WERE SHATTERED AND HE HAD TO WEAR THAT STUPID "PHANTOM OF THE OPERA"TYPE MASK FOR A LONG TIME!

Yes he did for a short time.
I believe that it was Yokozuna or Mabel that gave Undertaker several legdrops to the face that broke Undertakers bones. the Mask was awesome! It was dark grey and it made him look freaky!

Mark wrestled in a mask in the old Von Erich WCCW days as the Punisher. He started there around the same time as Foley.

Yes he had his orbital bone ( the bone around the eye socket) borken and wore a mask similar to the Phantom of the opera, this was around 1995-96

IN FACT HE DID! WHEN HE DROPPED "MABEL"(viscera) ON HIS FACE!!! HE EXECUTED THE MOVE WRONG AS MABEL WAS ON THE SECOND ROPE...BAM!!! HE DROPPED HIM! "TAKER" HAD TO WEAR A WHITE MASK FOR SOME TIME 'CAUSE HE BROKE BONES IN HIS FACE,THIS HAPPENED MORE THAN TEN YEARS AGO!THAT'S WHY HE LOOKS A LITTLE DIFFERENT THAN HE DID BEFORE.SOME PPL THINK IT'S A DIFFERENT UNDERTAKER BUT IT'S THE SAME ONE!

I don't think so....but i think he wore a mask as The Punisher...I'm not too sure about that. But he does wear eyeliner!!
