ask her directly and look into her eyes when you do. be firm about what you want to know, but don't overreact. if she is cheating or has cheated, ask her if she wants to continue the relationship with you. if you want to give her a second chance, do it, but be aware that you can't make people change (they obviously have to want it for themselves). if not, it's best for the both of you to go your seperate ways.

Unless you catch her , you can not tell. If there is some way that you can find out that she definitely is cheating you have a decision, you can confront her and if she want to work it out you can try or you need to find someone else. She gets to decide , not you if she is going to be faithful. You get to decide if her decision is one you can accept.
