The disc between two of the bones in your spine (vertebrae), has become squashed and has moved from the place where it should be.

Disc fragments separate from the originating disc and migrates beyond the disc space. Basically, the inside of the disc has pushed out so far that it has gone into the vertebral canal (the canal that houses the spinal cord).
