I read this expression in a novel by K. Vonnegut "... he was clean and foursquares."

I assume you're talking about "Slaughterhouse Five"...four squares is what the aliens refer to as the fourth dimension...most humans are unaware, or too ignorant to notice. So, being clean and foursquares was an expression to indicate the main character was much more than he seemed.

Basically the same as "foursquare." The meaning you want is in number 2.
