You use Roman letters (romanji). The order of the address is as follows:

Name
House number/ apartment number street
district (smaller than the town, the neighborhood)
town or city postal code
Japan

Here's an example:

John Doe
14-2-6 Nara Heights
Takachiki
Asuka-cho 632-523
JAPAN

I'm not sure if you made a mistake for someone in Tokyo or a big city, but even letters with some omissions got to me when I lived there. The Japanese Postal Service is outstanding that way.

When writing an address in Japanese, one follows a different style, but the Postal Service book I got when I lived there wanted the above. Start with the smallest entity, name, and expand to the largest, country.

Do ask the recipient how to write out the address.

just write it in English (romanji)
its exactly the same as you would address any other package.
I do it all the time, there is no problem.

PS. Dont forget to add Japan :)
