Just to add to mrz the blower motor switch, or resistor could also be bad. Another problem known in the GM market is sometimes the ground wire on the back of the AC control head gets a bad contact, you can check by pulling the Control head out, if the conector is melted aroun the black wire then there is the problem. If you have a test meter then you can check the switch for ohms on each of the four settings and you can check the blower motor resistor on each set of resistors.

This sounds like a loose connection or faulty resistor pack. It could be either at the fan, fusebox, or at the switch / resistor pack. Check the wiring connections at the heater fan first, check the fuse is seated properly at the fuse panel, then check wiring at switch.

Sometimes the plugs on the terminals on the back side of the fan under the hood get loose or are making irregular contact.
Check this out. Could have a little corrosion on them also.

Determine if it's the fan motor or loss of voltage to the fan motor. If you can physically see the fan motor itself, try tapping on it with a screwdriver handle to see if it starts up whenever you tap on it. If so, you may need a new fan motor.
