I mean in terms of back support while sleeping. Is the mattress the only real important thing, or do the bedframe and/or box spring play a big role also?

if the frame does not fit the mattress then the mattress wont have the proper support

The mattress is the most important factor. As long as the frame is firm I think you will be OK...
