Shemale = woman with a penis.

Born with a vagina, vagina soon turns into a penis over life.

"She-male" is simply a descriptive term for someone who is genetically male and through hormone therapy have taken on female characteristics such as breasts and a higher vocal range. These men may or may not choose to complete their genetic reassignment with surgery, and then they would be women, not men or shemales.

However in rare cases, a child is born hermaphroditic, and has the sexual characteristics of both genders, and is actually a third gender. This is not what is commonly referred to as a "shemale" as its a very rare occurrence, and usually resolved in childhood by the parent.
