When someone tells you to look in a direction by referring to time, they mean that you should look in the direction of that hour as indicated on the dial of a watch.

They assume that you are always facing 12 o' clock.

So 10 o' clock means you should look to your front-left and 5 o' clock means you should look to your back-right. Got it?

Usually when someone says (mostly in movies), "shoot, 10 O clock or 5 O clock", it means the attacker is on the side according to the clock, 10 would mean upper left, 5 would mean behind you on your right.

Clock ray method of indication can be done from two reference points - one where the clock rays emanate from the point where the Indicator is standing and facing and the second where clock rays emanate from a point where the person to whom the direction is being indicated is standing and facing or any other prominent reference point on ground.

In this case 10 O'clock and 5 O'clock indicate that an attacker is on my right rear and probably within striking distance. 10 O'clock for the person telling me about the attack and 5 O' clock from the position where I am standing and facing.

I will just pivot my right leg and swing right with left leg and right arm extended to hit the attacker's head. No doubt he will be felled.

Well if an attacker is at your 10 o'clock he would be in the left edge of your peripheral vision so I would have already double-tapped his head. Turn while crouching and fire, or if unarmed roll/run for cover.

But if you are referring to dogfighting i would probably accelerate and begin evasive maneuvers.
