it has been defined that way.
but those who defined it forgot to mention external factors that can change. so doing the same thing can perchance lock onto to something different even after thousands of attempts. Therein is the desired reward.

Depends on what that thing is. If someone was holding on to something or someone and doing the same thing over and over in hopes that eventually the outcome would be favorable I believe that happens quite frequently. If it is an addictive habit like gambling that is costing financially as well as emotionally then it's not insanity but is an action that might require professional help.

Not insanity, stupidity. Its like when I keep opening the fridge door hoping that what I feel like eating will appear.
