The elongated skull was just another variety of the human species. But it quite interesting how these particular people built some of the greatest civilizations know to mankind.
This fact also supports the theory of human migration and interbreeding across the continents by non-Europeans prior to the start of Western Civilization. I've read that some of these ancient civilations co-exisited and made contact with each other.

I studied the Maya for a long time (I am part Mayan and was studying Mayan Archeology) Some of the theories i have heard were...
In the Maya culture it was a sign of status and wealth. It was probably a way to differentiate between the classes. some one of noble birth could not be mistaken for a commoner or visa-versa.
some Maya also sharpened their teeth, so it is possible that it was a method used to make the person look more animistic as Part of their religious customs.
But one thing I personaly noticed was allot of the Maya have very long sharp features, it probably also became popular as to extenuate the natural features of the people.
I cant speak for the other cultures mentioned. Although I heard that some of the egyptians suffered from a genetic flaw that caused them to have elongated features and limbs, it is the same illness that Lincoln suffered from. But it was not very common and only apeared in one particular family.

Not sure what "orthodox science" is, but we would need some kind of proof that something occurred naturally, and it seems that most of the elongated human skulls found did not occur naturally or can not be determined to have occurred naturally.

Perhaps as some of the Northern tribes did they tied a board to an infants head until it took the flattened elongated shape desired.
Yes they did it on purpose to achieve a particular image.
I believe they considered it a mark of intelligence, status, and to some degree beauty.
