i love the question.
i recenty delivered a new 335i to a lady who was driving to virginia to give to her daughter as a surprise. When she picked it up, i had started it in the back and drove it to her. she never actually started it.
hours later, i get a call from her, at a rest stop asking how to start the car. Of course i had to tell her to place the fob in the slot, and put her foot on the brake, and press start.
same thing for the smg m5. key in the slot, foot on the brake, press start.

If it's an E60 (2004 to current - the new model), check out page 53 of the user manual (see link attached). If it's an E39 or previous (basically 2003 or before), you start it like any other stick shift car.

Make sure the transmission is in neutral, the parking brake seat, push the clutch to the floor and turn the key.

the clutch has to be depressed before you can crank it. If that dosen't work, try asking nicely. With the computers nowadays, who knows what'll happen.
