First, never take your eye off of the target until after you've thrown the ball. Next, when you let go of the ball, follow through with your arm. After that it's a matter of repetition or practice if you will.

That is your problem, you are trying to aim the ball. When you "aim" you do not throw correctly because your follow through is shortened and you throw tenantively.

If you are talking about pitching, do not try to throw it in a "key hole", instead throw it to the middle of the catchers body inside her shoulders. It is a bigger target to focus on. Start big and finish small.

If you focus so intently on a small target you will miss it more often than not, because it gets smaller and smaller than longer you stare at it.

I tell my pitchers to not pick up there target until they ge tin their balance points, it works better, mentally.

Bottom line, don't aim for the bulls eye but rather throw the ball inside the persons shoulder you are playing catch with and just let it loose and don't think about it.
