The heater core.
Prices vary, but usually around $100 or so.
Depending on the make/model of your van, you might be able to save some precious dough and install it yourself, though a second pair of hands is helpful. Is there a smell or any leakage of antifreeze? That's a dead giveaway that it's broken open somehow.

Could be:

1) Low coolant level.
2) Plugged heater core.
3) Stuck blend door in heater box.
4) Loose/broken controls.

the best thing to do is to make a 3 ft circle with some old bricks and build a fire in the middle and you will have fixed your problem
