It is six years old and in good condition with no previous moisture problems. The best idea so far is to use 1x2 pressure treated sleepers with expanded polystyrene insulation covered with 3/4 inch OSB. The 2-1/4 inch height buildup is not a problem. I am not sure what to do about a moisture or vapor barrier. How would you attach the sleepers?

there is a system available that comes in 2 foot square interlocking pieces. they have a plastic back and the top is osb. if you want to insulate the sleeper idea is good but the "r" value of eps is poor use extruded polystyrene or foil faced isocyanurate. if you use the extruded 250 series you could use a 2x4 on the seams and leave the field extruded I think you could get by with 1/2" underlayment over the top total would be 2".

If it is a finished basement floor, why not install radiant floor heat under tile? If you go with the sleepers you can glue them down with construction adhesive...Liquid nails.
