My bank charges about $30.00 a year rent for a box.
You need to identify yourself; social security number, drivers license will do it.

I think it's going to depend on which bank you go with. I have seen banks charge anywhere from $20 to as much as $50 to $60 per year

Depends on the bank. Call them or visit their website and e-mail them if the info isn't there. I pay $50 annually at my bank.
