If you think the water broke, you must go to the hospital now! Mom and baby need to be healthy and safe.
For most, it is just a gush of fluid, and maybe some blood in there. Occasionally there are women who just have a continuous trickle of fluids.
The water can break before the contractions begin, every woman's labor is different.

I remember when my water broke with my first child. It didn't hurt, I could feel it trickle. Labor (contractions) usually starts before or after and YES, your sister should go to the hospital once the water breaks.
Why didn't you just call the doctor and ask instead of asking here????
Now GO!

You will know as it feels like you are weeing and the water keeps running down - enough to need a towel to soak it up. Need to get her to the hospital soon as things can progress quickly from here.
Good luck.
