You can have only one appendix, as it is the singular.
More than one, and you use appendices, as it is plural.
If you have a collection of appendices at the end of a book, then each section is a separate appendix, but the collection are the appendices.
I recommend "Fowler's Modern English Usage" for resolving things like this.

They are both English words. Both are 'correct'.
You can have your appendix removed and have an appendix in a book.
You can only have one appendix removed. A book can have appendices.....more than one appendix.
This is in the UK English sense. US English may be different.
