I am confused now.. I thought you were asking whether or not I'd choose an old lady over a pregnant one or vice versa, but the first reponse interpreted it different.. so I'll answer both ways..

If in the situation I'd give my seat to an old woman or a pregnant woman.

If I had to choose between the two, it would depend on how old the lady was and how pregnant the woman was. I'd read the body language and decide on who I think needed it more. It would depend on the situation entirely. If the old lady was frail and probably needed it more than the fit pregnant lady, then yeah, the old lady. If the pregnant one looked hot/bothered and sick yet the old lady looked like she'd do alright standing compared to the pregnant one, then I'ld offer it to the pregnant one.
If I considered both needed it equally, then I'd probably offer it to the elderly lady over the pregnant one, just because I've been taught to respect my elders..

If both the old woman, and the pregnant woman were there at the same time.I would give my seat to the old woman.Because of respect, and older people may have more health problems.
