If you see that your belly starts to grow, yea! the condom got broke.
Just kidding, always that you are doing it with condoms if you feel like an edge in your partner's penis tell him to stop an take a look of it, probably is torn.

Generally you can tell if you find a hole and/or there is seminal fluid leaking out. Another way is if the girl gets pregnant then it is pretty sure.
