Most of the times you want to run it. It will start the set up program and install it. If you just save it it will only be saved to your computer and not install the program until you open the file which is saved.

save the dowload onto your computer. once done, double click and launch the application--it will execute a setup and install iTunes onto your computer. once installed you can get rid of the downloaded file.

Save it to where you want it.
Then run it later when you're ready.
