Yes, unless there is a complete bottom on the frame.
You can use a sheet of plywood to lay the mattress on

Yes, you need a boxspring. Make sure the metal frame will support the weight of the mattress and boxspring. We had a beautiful headboard and footboard, and had to replace the mattress and boxspring. We bought an expensive mattress and boxspring just over $2,000. on sale put in on our metal bedframe and because of the weight of the new set our metal frame could not hold it properly it actually made the mattress sag. It is not because of our weight as we are small and light people. If you don't want to buy a boxspring then the mattress has to be placed on, what I call a captain's bed. Full platform bed, which is solid on all sides and bottom, this way it will support the full weight of the mattress plus the people and not cause undue stress on the mattress.

For the type of frame you have shown, you need to support your mattress with something... a box spring is made to do this.
One other alternative, is to build a platform to support your mattress. Use 2x4 lumber, and at least 1/2" plywood. Throw a layer of 6 mil plastic over it, and it will keep the mattress from picking up any splinters.
If you toss your mattress on that frame you have listed, its only going to fall thru the open areas.... hehehe... you'd be better off just putting the mattress on the floor than that.

You definitely need a box spring for your mattress.
Think of it this way: You "can" have a car without shocks. But, the damage it will do to your car will not make it worth it. Not having a box spring will cause extreme sagging and the life of the mattress will be MUCH shorter.
A box spring will support the mattress and it will not sag. Most mattresses will void their warranty as well if you do not have the proper box spring under it. (and when you are paying good money for a mattress, you want to get what you pay for)
The only way you can "not" have a box spring with a bed is in the case of a platform bed.

With the particular frame that you made reference to, yes, you need something to support the mattress.

If you use anything else, why have a frame when putting it on the floor does the same thing. This frame was made for box springs.
