Tuberculosis, I believe. That is the only disease I can think of when you cough and spit up blood, and it was rampant at the time period that Moulin Rouge takes place.
I love that movie.

She has Tuberculosis, which was often referred to as "consumption" in the 1800s and early 1900s.
