Check the MLA and the APA. If you want to do parenthetical citation in addition to a "Works Cited", I'd suggest using http://www.citationmachine.net to get you started.

Depends on the book. Usually can have a bibliography at the end. If scientific, cite each reference and then have a references section at the end.

The go at the end of the book.. in a section labled bibliography..

you put the book title, author, publisher, year, and page number as reference for each source...

It depends on the style of writing you use, and what type of book you are writing.

If you are using MLA format, you have the option of using parenthetical citations, footnotes, and end note. Same for ALA. Just look the different formats up, pick which one you like the best, and go with it.
