It can be either. In my mind, anybody who dresses up in order to 'spice up' their relationship, who just dabbles in it, is probably using it as a form of foreplay. It's the kind of thing where you don't find nurses or cops normally that sexy, but when you're wife is dressed as a sexy nurse or your boyfriend dons his cap 'n' badge, you get a little hot at the naughtiness of it and the situation. Others, who find extreme pleasure in playing these games, especially when the clothing symbolizes power structures and not just fake scenarios to play out like doctor and patient, but the power imbalance between cop and victim, for instance, or sergeant and soldier, I'd say have a fetish for it.
I know that there are some purists who would tell you that it cannot be a fetish, as fetishes place sexual desire onto objects- for instance, the wearing of rubber might be a fetish, or you could have a fetish for cop outfits, but not really the roleplay behind it, which is something else. But I'm not a stickler, so I can say some people do have a fetish, which, to me, means that they don't use it as a means to sexual desire, but as sexually arousing in itself.

It can be both, but usually dressing in a uniform for pleasure is considered a fetish, which is fine, and fun, and many people enjoy this kind of play....
and some couples roleplay as a prelude to sex by dressing up and playing games. Its part of the wonderful world of experimentation.
Dont worry about the labels, just enjoy what feels good between consenting adults.

Depends. And it depends on how you're defining those words. It only becomes psychologically abnormal if the item is necessary for sexual pleasure. If you're more attracted to the uniform than you are to the person in it, there might be some problems there. But as for the more vernacular use of the word "fetish", I guess it qualifies as both.
