What I have always told my friends to use and it always worked, is powdered Alum. You will find it in the spice aisle at the grocery store. Not very expensive. Used in pickling sometimes. Make sure to buy the powdered, not the granular. Wet the tip of your finger, dip it in the Alum and dab on your cold sore. Repeat as needed. What it does is allows the cold sore to dry out so it can heal. Taste like crud, will make your lips pucker, but it works. Make sure to keep this Alum in the medicine cabinet. And mark it medicinal or something so as not to confuse with regular spices.

Take an ice cube, wrap it in a wash cloth. Hold this to the cold sore. It will help keep the swelling down. Do this often but not so often that you will frostbite your lip. Get Abreva from your local Walgreens. It's over in the section where Chapstick is sold. Apply often throughout the day. I've had these all my life. Keeping the swelling down; this seems to help most of all. It keeps it from developing into a sore that is hard to heal because the mouth is always moist. Last, don't let your mouth/lips get dry. Keep them moisturized with lip balm.

Cut a fresh lemon and put over the cold sore. It hurts like hell.
After you peel yourself off the ceiling, you'll find the cold sore will disappear very quickly. It works better than anything else I've tried. But it does hurt a lot for a few moments.

My parents would usually tell me to gargle with warm salt water, patch a wod of salt directly on the sore for a couple minutes....
Hopefully that will help...but it may sting with the patch of salt if it's an open sore.
