You first need to make sure that you truly lover her. If she is afraid that you will hurt her again, then you really need to show her why she is wrong to think that.
Then you need to explain to her that you were mistaken before and you want a permanent relationship with her. You have to tell her and show her that you are totally sincere and truly want her back.
