Yes, you can call a fair catch on a kickoff in the NFL. You don't often see it, though. If the ball goes into/past the end zone, it comes out to the 20-yard line. But if it's touched in fair territory, its down wherever the perosn who catches goes down. So the idea is to catche the ball in fair territory only if it isn't likely to go into the end zone--but once it is caught, there's rarely an advantage in calling a fair catch--in fact the opposite is true.

Yes you can. This is why it is important on an onside kick to kick the ball into the ground. If the onside kick is pooched, then the receiving team can call for a fair catch and the recovering team can't impede on the receiver.

I think the person that receives the ball has to step back into the endzone and go down on one knee. The ball is then started into play on the twenty yard line.
