It depends how well you know them and/or what kind of relationship you have. With my husband, I just come out and say, "You need a breath mint," or "Did you brush your teeth?"
If it's a co-worker or something, it's a little trickier. I have actually come out and told my officemates in the past that if I ever have that sort of problem, I really, really want them to just tell me! It's always better to know and be able to do something about it than to just go around stinking up the place and having people talk behind your back.
I think you would need to take the person aside in a private situation. You might say something like, "I'm sorry if I'm embarrassing you saying this, but I would really want someone to tell me--you have a problem with your breath."
DON'T make jokes, and don't do silly things like leave deoderant on his desk. That's just humiliating someone for no good reason, and it's not the best way to deal with the problem.

That is a terrible situation to be in. I guess you should put yourself in the other person's shoes. Think about it. If YOU had bad breath or b.o. would you want somebody to tell you? I guess it would be okay if somebody who really really loved me told me that - because I would know that they were doing it for my benefit and not to be cruel.
So - if it was somebody who I really loved I would tell them - very gently. I would tell them that I'd noticed it - and that I knew that sometimes people can't tell themselves that they have bad breath or whatever - and ask what I could do to help.
Sometimes people shower and brush - but odors still persist. If you don't really care that much about the person but KNOW somehow that they are downright inconsiderate about tending to their hygeine - I guess it's okay to mention it - in a kind and considerate way. Otherwise - telling somebody these things for any reason other than their own well being is cruel.

It depends on how well you know them. I would just pull them to the side and kindly tell them they may have some secret you may not know about!! I have a friend who can't wear deodorant because of allergies. You may be surprised! Just say with a smile on your face that you are only telling them this to help them and not embarrass them.
