The skin over ones lips is relatively thin (only 3 layers thick) as opposed to facial skin that is 16 layers thick. Too, lip skin contains no melanocyte (pigment cells which give skin its color) plus red blood vessels appear through this thin skin. That said, the lips of a fair-skin person appear a lighter red color than those, let's say, of a much darker-skin person. In exceedingly darker-skin people, you can see through the thin layers to darker tissue which contains more and more melanin.

Here are a few reasons for the difference:
1. Genes
2. Temperature: cold lips are not as rosy as warm ones
3. Anemia: If you are anemic from heavy periods, other blood loss, iron deficiency, etc., your lips will be a paler pink instead of red.

genes blood flow i dont know i just know that i look like a coked out snow white after ive been out drinking really dark red lips and the rest of my face is pale except for the dark circles under my eyes

I have red lips too and I can accurately say that one major reason is lip suckling (biting) its one habit I didn't outgrow and it contributed to my red lips..
