Get "Shoe Goo" at someplace like Walmart or Kmart. Use tape to create a dam on the heel, then squeeze on the goo to replace the worn away rubber. Set the shoe so the goo will be level, and let it set up completely.

they make replacement heel plates you can nail onto the heel.
