It sounds like you know what you're doing, but I can't tell from your question if you have a serious problem or if it's just Windows XP hiding unused shortcuts.

If you haven't done it already right click on Start, Select Properties, Click on Task Bar and make sure Hide inactive icons is not checked.

One important step you didn't mention in your clean up is did you do "Check Disk?" If not Double Click on My Computer, Right Click on the C Drive, Select Properties, Tools, and next to Error-checking, Press Check Now, Check Fix and Attempt, Press Start Now. It won't be able to do the C Drive while windows is running, so Click Yes so it will do it next time you reboot. Reboot your PC as soon as you can.

I had a problem like you describe after running a boot time defrag with an old version of Diskeeper. Mine quickly got worse where all shortcuts in folders disappeared. Unfortunately, I wasn't able to find a fix and eventually rebuilt my PC.

Hold the mouse cursor at the bottom edge of the screen and use the resizer to pull it up. If this is the deal, right click on the taskbar, go to properties and deselect auto hide.
