Introduction is just a matter of greeting and shaking hands, sitting where the interviewer indicates, and smiling in a positive and friendly way. The interviewer will take the lead.
Why you left the previous job is one of those tricky ones. Why did you? If it was because your former boss was a jerk of the first water, I don't think you better say that. You might go so far as to say you did not feel you were appreciated (loosely translated: no pay raise or no promotion), or that you felt somewhat stifled by the office politics (loosely translated: you lost in office politics).
People know there must be negative things about your last job, or you would still be there. All they are really looking for is whether the problems were about you or about the company. Still, it is traditional that you do NOT say bad things about your former boss. So what you need is a packaged euphemism to cover over this breach in etiquette vs. honesty. Something like "I was beginning to stagnate there, and felt like there were too few opportunities for advancement." This could mean that your boss was personally set against you, or that you were in too small a firm and there was no position above you that you would even want. It is up to the interviewer to figure out which.
I had this problem in moving from a courtroom clerk position to a private law firm. I wanted soooo much to say, "because I hate working for the government," but felt that would sound too rebellious. I also had some major personality issues with my immediate supervisor, but I was told never to say anything bad about a former boss, so I hedged on that.
But in a small town, everyone in the legal community knows everyone else anyway. Fortunately, the interviewer not only knew the supervisor I was so set on getting away from, but had even witnessed her vile temper in the courtroom and in the clerk's office himself on several occasions. He mentioned to me that he understood my wanting to get away from there.
