what happens at the end of the anime gravitation???

i watched the 13th episode but it wasnt subtitled so i didnt get one word..
so basically i jus want some one to explain what happened.
the part where shuichi comes to yuki in that cute dog costume of his what does he say to yuki? and in the very next and last episode shuichi goes back to his concert and then in the ending credits yuki visits the graveyard..what was he saying?

but what i badly want to kno is if shuichi and yuki are finally really together?
