I am afraid you have to remove it from its setting. Testing a diamond for impurities, minor fractures etc cannot be done unless it is removed from its setting. At the best, you can get a preliminary idea of the gem when it is in its setting.
Scratching glass is never a good test for diamond, because even quartz will scratch glass. Moreover, sharp points may break to spoil the gem. Same is applicable for "water immersion test". A good gem testing lab will never accept a gem in its setting. In its setting you cannot even determine its specific gravity.
This is why one should perform all the tests before putting a gem in its setting.

It depends on what you mean by testing a diamond. If you only require the simple answer to is it, or is it not a diamond then there is absolutely no need to remove it from its setting. However if you want to check the clarity, colour and weight then the stone must be removed as there is no way these qualities can be determined when in a setting.

You can get a good general idea even if the diamond is IN the setting but if it is to be REALLY tested, I would certainly take it OUT of the setting to do so.

No. You can test it by submersion in water. In performing this test, the diamond will show as brilliant and each facet edge will remain visible. This is also true for cubit-zerconia. Another test is to scratch glass. A diamond will scratch the glass, cubit-zerconia will not. These are simple tests for which the diamond can remain in its setting.
