1) Failure to keep right (in the USA)
2) Not knowing how to merge onto a highway
I suspect that #2 is in no small part responsible for #1; However, on a 3-lane highway with light traffic, there is NO reason to stay in the far left passing lane if you're not passing!

3) Failure to use mirrors
4) Failure to yield to passing traffic
These two are frequently committed by those violating #1 above.

5) Speeding up for a Yellow light +/or Running a Red light
6) Cutting into a line of traffic that is waiting for an exit/turn, and they pass the line and then try to squeeze in (didn't these people attend Kindergarten and learn to wait their turn?)
7) Flicking lit cigarette butts out the window, when they bounce around and can get into someone else's car. I have had them bounce up to my grill and I get stuck smelling that $#!+, or having them bounce onto my windshield and I reflexively duck.

Ooh pet peeve time. Failing to use a blinker, either when turning, or, changing lanes. Failing to turn a blinker off, especially when one is on a straight away highway. Speeding to get ahead of you, then slowing to a crawl once they're in front of you. Being cut off. Failing to move over so that people can merge onto the expressway. (we aren't being rude you know, we just need to get to work). Number one: failure to wave a thank you when I allow you in the lane of traffic ahead of me. Stop acting like I'm you're servant sent to do your bidding and make your life easier. By the way, changing lanes is not a sport, especially at high speeds.

Switching lanes w/out signaling. Tail-gating. Driving too slowly in the passing lane on the freeway. Not paying attention when a light turns. Running stop signs and/or red lights. Fiddling with makeup, cell phones, food. Not having kids in car seats. Not buckling seat belts. Throwing trash out car windows. See all these way too often!

Making a left or right hand turn around the world. Please turn off that turn signal. What kind of car does a person drive that they cannot hear that constant ticking that the light is on?

Cutting in front of others in a different lane and not signaling that they're about to do so. Bad!

I have to say the cell phone thing... even using headsets, drivers are still distracted from where their attention should be.

One hand steering, one hand chatting on the phone is a norm in cellphone-mad Asia.
