Choose a research topic i.e. 'fear of crime'. Then sub-divide this into some topic headings i.e. 'personal safety; home security; etc. etc. until you have as many headings as you can. Then for each sub-heading, put together three questions (one behavioural i.e. how would you behave if; one cognitive i.e. how would you think if; and one affective i.e. how would you feel if..)

Then you would ensure that 50% of the questions are positively phrased and 50% are negatively phrased (this is called a Lie Test).

You would probably try to use something like a Likert Scale, whereby you would have 1 as agree and 5 as disagree with 3 being neutral (although people tend to sit on the fence if you have an odd number).

You would then run a pilot to test out the questions and run some statistical tests to eliminate questions that are not being answered in the expected manner.

Dependant on the style of questionnaire, multiple choice, q & a, rating i.e. 1 being good 5 being bad.
Decide on format then set up accordingly using a spread sheet.
Question's in column a , headings in row one etc

depends on the type of questionnaire and answers you want. If you want qualitative or quantative meaning will your answers be represented by figures or information.....you can ask direct question with yes no answers which you could them turn in to figures like a tally chart or ask open questions (who, what, when, where, why and how)....write the questions so they are appropriate for the group you will be targeting...there should be lots of this info on the internet....
