He has mouth fungus.
Go buy an anti fungal treatment and some aquarium salt.
Change 100% of his water, add the drops of medicine and a pinch or 2 of the salt. Do this daily for around a week and he should be fine.
Melafix won't cure a fungal problem. The salt will perk him up.
He got the fungus because you aren't keeping him clean enough. Change his water weekly or more and remove any food he doesnt eat.
ps he is a SIAMESE fighting fish also known as a betta splenden.

This site recommends the proper care as follows:
STEP 1: Buy koi pellets to feed your fish. Other foods they can eat include shrimp (fresh or dried), lettuce and cooked rice. STEP 2: Feed your fish one to two times a day, depending on the season. (Once a day during the winter, twice during the summer.) STEP 3: Be sure to keep the water aerated and filtered at all times. STEP 4: Avoid sudden temperature changes in the water whenever possible. STEP 5: Remove any debris from the floor of your pond. STEP 6: Use a chloramine remover if you add tap water to the pond. STEP 7: Clean your filter two times a year, even if the water looks clear. STEP 8: Take care not to strip the protective slime from your fish if you handle them. This coating helps them resist fungus and bacteria. STEP 9: Change water in the pond gradually to avoid sudden temperature changes. STEP 10: Remember that as koi grow, they will need more room and more oxygen. In anticipation of this, try not to overfill the pond with too many fish.
Koi can survive for close to two weeks without eating anything.
Know that if your pump ever breaks, you should hold off feeding your koi, since they use more oxygen up after eating.
Avoid sudden changes in pond temperature and other severe conditions; the koi may not survive the resulting stress.
If you have been doing these things, you may want to consult the pet shop where you got him or any other local petstore that carries them.

Your fish will be feeling very poorly. He needs some medicine from the pet shop.

do you mean siamese fighting fish? What are the tank stats? PH levels. ammonia and nitrate levels etc. It has a fungus infection so you need to medicate the water, then go and find out how to care for fish properly.

go to the pet shop and tell them they have all types of medications for fish i had to get one for my sick beta it works. change the water daily and get a thing to clean the amonia and lead out of tap water and helps the fish live longer. ask the pet store for any additional things. GOOD LUCK!!!

Maybe its white spot aka Ichthyophthirius. Have you changed the water put a new fish or plant in recently? Its easily treated. See below
