The only info you should need to report them is their IP address and the time of the attempted hack. Report that info to your ISP and they should be able to track down the offender.

There's no correlation between IP addresses and street addresses. Your ISP can link your IP to the service address but they don't release that information without a subpoena.

If you left VNC Server running with a weak password, you deserve to be hacked! Or if you're running naked (no firewall) it's pretty easy to attach to your registry remotely and crack the VNC password.

You can report the incident to your ISP but you will need logs to back up your claim. VNC does log connections by default so you can export those from Event Viewer and they should suffice. The ISP needs the EXACT time of day that it occured, preferably down to the second. Make sure that your computer is synching its time with a reliable source or those logs will be useless.

You cannot obtain that information yourself. Only a federal or area police agency can get a home address through a court order presented to the ISP who will then identify the individual.
As for the hack you are experiencing, it may be that you have downloaded a trojan through a file sharing program. (R u using Limewire or a similar prog?)
However, since you believe it is through your ISP it may well be that you have seen innocent activity such as software updating or or simple pinging or whatever and have misinterpreted what is going on.
Just run a good virus scan and if nothing is detected chalk it up to paranoia!
I have seen many examples of your reaction and almost always found that my customers were just misinterpreting normal computer activity.
