This is not a "grammatical" error like misusing or misspelling a word. It is rather called "redundancy." It makes for boring reading to see the same word repeated.

the inspiration to write the Bible was given to humans that had no great Scribe skills but zealous hearts for God.The Bible isn't an english lesson book but a history of and prophecy for mankind to find the true God and live for ever.I look for the deeper things in the message and don"t worry about the small imperfections that might have occured through God's secretaries.God invented languages and he knew every living person if he had been concerned about grammer he could have had the experts of the given times write the bible.Scripture says to be cautious of the rafter in your eye before removing the splenter from anothers eye.

Accidentally is the operative word in your question. In that case its an error. Grammatical and potentially typographical. So, yes, yes it is and it's it's not.

Yes, that is a mistake, not a redundancy. Redundancy breaks no grammatical rules, just rules of style.
"Your question is a question that deserves an answer." has a redundancy, but is not grammatically wrong.

No, there are those occasions where it is the best way to get the point across for those with less grammatical knowledge.
