How is it that pushing a pedal and advancing additional fuel makes a vehicle accelerate? How does it know to begin producing more output?

Think of an engine as a huge airpump. The butterfly on either the carb or efi only allows a certain amount of air into the cylinders. Hypotheically, if there was no butterflys, the engine would run full blown. The butterflys is what keeps the engine at idle. The more air you introduce into the cylinders, the more air that is compressed and the more power that is created. That is why if youcompletely block the carb or efi intake, the engine sill stall because that is not enough air in the cylinders to compress and create power.

Although you can increase the fuel, you can't change the stoichometric ratio (the ratio of air to fuel in the cylinder), which under normal conditions is 14.7:1. As you increase the amount of fuel, you increase the volume of air, ensuring all the fuel is burnt. Maintaining the ratio is done by the fuel injection/engine management system, or the carburettors on older cars.

The more fuel burnt, the greater the energy applied to the crowns of the pistons, causing them to accelerate faster, which then accelerates the rotation of (in order) the crankshaft, flywheel, gearbox, driveshafts and eventually wheels.

An accelerator.This is a device in the car that is attached to the engine that when you place your foot on the gas it increases the amount of fuel to your engine and this causes a reaction which allows the engine to increase it's motorization.
