It means "In a bold aggressive manner."
" This originated in the US in the 1970s. Many of the earliest citations relate to confrontations in sport. The earliest I can find is from Charles Rosen's novel about basketball - A Mile Above the Rim, 1976:

It means that someone is being overly aggressive or confrontational. I would guess it comes from the image of someone invading your space by putting their face right up to your face. It is not usually literal, however, it is usually a figurative expression.
