It depends on how old the exhaust is. If its a few years old I'd probably be inclined to do both as all that will happen in a few months time you'll end up having to replace the middle section anyway.

Not worth two trips to the garage with associated hassle to save just a few £££'s for a short period.

No you don't but if its the original factory fit exhaust that needs replacing the centre section and back box are all one piece so it will have to be cut in the exact right place to remove the back box section so the new one fits.

If by the "back box" you mean the muffler, then no. As long as the rest of the exhaust is in good condition (no holes or major rust) then its fine. You should make sure to remove any rust off the tip of the pipe where the muffler attaches though so it doesn't start rusting your new muffler. If it is too rusty, it may need replacing or repairing (repair is usually fairly easy)

Not if it's in good nick. If it's the original, it will probably be all in one - the fitter will know where to cut it to fit the new back box. if you go to Ford, they'll probably try to flog you an entire set.

You don't have to, but it's often worth doing at the same time.
