The character who says that line is strung out on drugs, and so messed up that he can't tell it is a man talking to him, though at some level he understands that the man talking to him is a benign person. But he is completely oblivious to the content of the message. It is highlighting how drugs affect people's ability to function and communicate, though it may make for good drama.

Because he is supposed to be so messed up on drugs that he can't even tell that the guy talking to him is a guy. The point of the commercial is that the guy is telling the junkie that the show he is on is not apprpriate for his kids and he is now blocking the program.

Because he's suppose to be drug addict, who can't tell the difference, because he's high.
