As I understand it, it's because our uterus contracts (as in labour) to expel the endometrium that has built up during the month. I also get diarrhea, as a contracting uterus often triggers bowel movements, so I get cramps on both sides. Yay :)

The uterus contracts to release the build-up or lining when there is no pregnancy.
The muscle contractions are more painful in some people than in others.
