It depends on what you mean by budget form. You can create forms in Microsoft Word, Excel or by Using Adobe Acrobat Professional Designer.

The design and body of the form would include what aspects of budgeting are important for you. Such as technology improvements, cost analysis, ROI, etc.

A budgeting form could also be a list of technology requests and their amounts for any one fiscal year.
