It part of the "sexy" equation. They do certain thing for optical illusions. They are little tricks to give slim looks. Think of the cliche 'the camera adds 10lbs!' The high heels help give the slender look because it helps to give the legs a longer look. The heels position a woman's body to help poke out her chest too. Also if you notice a lot of women have still have large full hair. This takes away from round faces. The women who have long straight hair, usually have long faces too. With having the woman looking smaller, this makes the man's penis look bigger.
I believe too that the high heels adds to some people's fetishes. Some men/women are into feet and like those types of shoes on them. It how (s)he get aroused.
Another reason could be height. Since this is a fantasy world, sex is not always in the bed so to help with positioning, you may need extra height at times for a little comfort.
