It is usually recommended that one has the tires rotated on their vehicle every 6,000 miles (or every 6 months or every other oil change). this is to even out the wear on all the tires so that they last longer, and that the vehicle does not pull to one side or have other steering problems by all the wear being on one particular part of the tire itself.

Every other oil change (around 6000 miles). Believe it not it really does make a big difference in the way your tires will wear.

4 times a year if you drive a lot, twice a year if you dont put too many miles on the car.

Look at your car's manual. They'll tell you in there.

As a rule of thumb, about every six months or so.
